Instructions:

2) Request a New Patient appointment. Please provide at least 3 options, on 3 different days.
3) Give credit to who referred you AND Type the word: PROMO. Click SUBMIT.
3) Look for a response from our office in the email you provided.
4) Confirm your appointment by registering for the Patient Portal and completing the paperwork which was sent to your email.
5) Bingo! Your Chiropractic Journey to a Healthier Lifestyle has begun!
6) As long as you are new to our office, you now have 4 visits of Chiropractic Care on your punch card, including all evaluation procedures. (Expires in 45 days).
7) PLEASE REFER YOUR FRIENDS AND FAMILY.
